Direct Primary Care is not comprehensive health insurance. Many members keep separate coverage for specialist care, hospital services, emergencies, and needs outside primary care. Uninsured patients may also enroll.

How much does membership cost?
The current published rate is $75 per month for an individual or first family member and $35 per month for each additional family member. There is no enrollment fee.
What is included with membership?
Membership includes primary care access and, when clinically indicated, blood glucose testing, urine pregnancy testing, rapid strep testing, urinalysis, suture or staple removal, and ear irrigation for cerumen removal.
Can I cancel membership?
Yes. Current membership guidance states that you may cancel at any time and that the final month is prorated.
Can I use insurance instead of joining the membership?
Yes. Freedom Primary Care offers insurance-based primary care as well as Direct Primary Care membership. Membership is not the only way to become a patient. Contact the office to confirm your specific insurance plan and discuss the care option you prefer.
Can I keep my insurance if I choose DPC?
Yes. Direct Primary Care membership is a separate payment model for primary care, not comprehensive health insurance. Many members keep insurance for hospital care, specialists, emergencies, and services outside the membership. Do not assume your insurer will pay the membership fee; confirm any benefits with your plan before enrolling.
How do I confirm whether my insurance plan is accepted?
Call 860-245-1791 with the exact insurer and plan name shown on your card. Ask the office to confirm network participation, then check your benefits, copays, deductible, and service coverage with your insurer. Participation and benefits can vary by plan and may change.
Which insurers do you accept?
The practice’s accepted insurer list includes Aetna; Anthem / Blue Cross Blue Shield; Cigna; Humana; Medicaid (Husky in Connecticut); Medicare; TRICARE; UnitedHealthcare. Acceptance may vary by specific plan, product, and network. Contact the office and your insurer to confirm participation and benefits before your visit.
